How it works
The timing rules behind the dates
- 5-day Medicare assessment: reference date on days 1 through 8 of the stay (days 6 to 8 are grace days)
- Admission assessment: reference date and completion no later than day 14
- Quarterly assessment: reference date no more than 92 days after the last OBRA assessment reference date
- Annual assessment: reference date no more than 366 days after the last comprehensive assessment reference date
- Quarterly and annual assessments: completed within 14 days after the reference date
Why it matters
Late or missed assessments affect payment, quality measures, and survey findings. A missed 5-day assessment can mean days billed at the default rate.

When is the 5-day Medicare assessment due?
The assessment reference date must fall on days 1 through 8 of the Medicare stay, with the admission day counted as day 1. Days 6 through 8 are grace days.
When is the OBRA admission assessment due?
The assessment reference date and completion must be no later than day 14 of the stay, counting the admission date as day 1.
How often are quarterly MDS assessments due?
The quarterly assessment reference date must be no more than 92 days after the reference date of the most recent OBRA assessment.
When is the annual MDS assessment due?
The annual assessment reference date must be no more than 366 days after the reference date of the most recent comprehensive assessment.
