QAPI & performance improvement

QAPI programs that drive real improvement

Our QCP-certified consultants build CMS-compliant QAPI programs that prevent citations and improve resident outcomes, not just binders and meetings.

CMS framework

The five elements of QAPI

  1. 01

    Design & scope

A QAPI program that covers every service and department in the building.

  2. 02

    Governance & leadership

Leadership oversight, accountability, and resources for improvement work.

  3. 03

    Feedback, data & monitoring

Systems that collect and trend the right data from staff, residents, and records.

  4. 04

    Performance Improvement Projects

PIPs chosen from your data, run with clear aims and measures.

  5. 05

    Systematic analysis & action

Root cause analysis and PDSA cycles that change systems, not just people.

Survey readiness

What strong QAPI looks like to a surveyor

Surveyors look for evidence that QAPI is a working system: the committee meets, uses real data, chooses Performance Improvement Projects from that data, and changes practice as a result.

The most common weakness we find is a QAPI binder that documents meetings but not decisions. We rebuild programs around a short list of measures the leadership team actually reviews each month, with clear owners and dates.

  • A written QAPI plan that covers every service in the building
  • A data dashboard reviewed at every QAPI meeting
  • Active Performance Improvement Projects with aims and measures
  • Root cause analysis for adverse events and repeat tags
  • PDSA cycles that test changes before they are rolled out
  • Minutes that record decisions, owners, and follow-up

Process

How a QAPI engagement works

  1. 01

    Program review

We review your QAPI plan, minutes, data, and recent survey findings against the five elements of QAPI.

  2. 02

    Data and priorities

We help leadership choose a small set of measures and the two or three problems worth a Performance Improvement Project.

  3. 03

    Project design

Each PIP gets an aim statement, measures, a root cause analysis, and a first PDSA cycle.

  4. 04

    Coaching and monitoring

We coach the committee through the first cycles and set up monitoring that continues after we leave.

FAQ

Frequently asked questions

What are the five elements of QAPI?

Design and scope; governance and leadership; feedback, data systems, and monitoring; Performance Improvement Projects; and systematic analysis and systemic action.

What is a QAPI Certified Professional?

A QAPI Certified Professional (QCP) has completed AAPACN’s QAPI certification. SMK Medical’s founder, Dr. Demetrius Kirk, is an AAPACN QCP Master Trainer.

How many Performance Improvement Projects should a nursing home have?

CMS does not set a number, but surveyors expect active projects chosen from the facility’s own data. Most buildings do best with two or three well-run projects at a time.

Can QAPI reduce falls?

Yes, when it targets root causes. Dr. Kirk’s doctoral project reduced falls by 86.6% using the Morse Fall Scale within a structured improvement process.
