Where to start

How to choose the right service

Start with the risk in front of you. If a survey is open or just ended, start with survey emergency response. If your window is coming up, start with a mock survey. If revenue or Five-Star measures are slipping, start with an MDS and PDPM review.

Most facilities combine services over time. A typical path is a mock survey to find the gaps, a Plan of Correction or QAPI project to fix them, then MDS auditing and staff training to keep the fixes in place.

  • Survey window opening in the next 90 days: mock survey or SurveyPrep™
  • Statement of Deficiencies (CMS-2567) in hand: Plan of Correction support
  • Immediate Jeopardy cited: call (313) 314-8267 for a removal plan
  • MDS coordinator out or behind: 48-hour MDS nurse coverage
  • Repeat tags or weak QAPI: QAPI program reset and PIPs
  • Closure, ownership change, or leadership gap: transition support

FAQ

Frequently asked questions

What does a long-term care compliance consultant do?

A long-term care compliance consultant helps nursing homes and skilled nursing facilities meet CMS requirements of participation and state rules. At SMK Medical that means mock surveys, survey response and Plans of Correction, MDS and PDPM accuracy, QAPI programs, staff training, and transition support.

Do you work onsite or remotely?

Both. Mock surveys, Immediate Jeopardy response, and training are usually onsite. MDS coverage, Plan of Correction writing, policy work, and SurveyPrep™ are usually remote.

Where is SMK Medical located?

SMK Medical is based in Detroit, Michigan, and serves nursing homes and post-acute providers in all 50 states.

How quickly can you start?

Survey emergencies can often be supported the same day. MDS coverage starts within 48 hours of a signed agreement and EHR access. Mock surveys are scheduled around your survey window, usually within a few weeks.
