Immediate Jeopardy can cost more than $10,000 per day.

Plans of Correction that get accepted

A Plan of Correction is a legal document. It has to address each deficient practice, the residents affected, the systems you are changing, how you will monitor, and who is responsible, with dates you can actually meet.

We write Plans of Correction with your team, not for them, so the corrective actions match what your building can sustain. Our founder also built SmartPOC®, the Plan of Correction module inside SurveyGuard.

  • Root cause analysis for each tag
  • Systemic corrective action
  • Monitoring and QAPI integration
  • Allegation of compliance timelines

FAQ

Frequently asked questions

What should a nursing home do after an Immediate Jeopardy citation?

Act the same day. The facility needs a removal plan that stops the immediate risk to residents, staff education, and evidence the plan is in place. SMK Medical can deploy onsite within 48 hours and supports removal plans by phone immediately. Call (313) 314-8267.

How long do we have to submit a Plan of Correction?

Facilities generally have 10 calendar days from receipt of the CMS-2567 to submit an acceptable Plan of Correction. Confirm the deadline in your state survey agency’s letter.

What makes a Plan of Correction acceptable?

It addresses each deficient practice, the residents affected and those at risk, the systemic changes made, how the facility will monitor and for how long, who is responsible, and a realistic completion date.

Can you help with informal dispute resolution?

Yes. We review the CMS-2567, assess whether findings can be disputed, and prepare documentation for informal dispute resolution.

CMS isn’t waiting. Neither should you.
